How long are ‘enduring’ permission forms good for?

‘Enduring’ permission forms are a popular options for many church kids and youth groups.
They are comprehensive permission forms that are good for more than one outing or event. We have an example available here.

But how long can these forms ‘endure’?

We strongly recommend that you generate a whole new form for each child once per year.
The main reason for this simple: lots of personal details can change in the course of 12 months, and so renewing the permission forms every 12 months is a great way of making sure that contact details, medical information (including dosages of medication), ‘prohibited’ people information are all current.

Forms should be retained for a period of time (4-5 years is reasonable) in case any information on them is required for a future investigation.
