The form below should be completed by the Senior Minister of the church when:
- A Safe Ministry Rep resigns – and you have another person ready to appoint
- A Safe Ministry Rep resigns – and you DO NOT have another person ready to appoint (in which case the Senior Minister becomes the SMR)
Please complete all fields and submit as soon as you have all the relevant information
